Job summary

San Andreas Regional Center is seeking a dedicated professional to support individuals with developmental disabilities and coordinate services under the Lanterman Act. The role focuses on assessments, planning, advocacy, and collaboration with families and the Interdisciplinary Team.

The position emphasizes client-centered planning, crisis intervention when needed, and ensuring access to eligible services while maintaining compliance with state regulations.

Qualifications

• Bachelor’s degree in social work, rehabilitation counseling or related field.
• Ability to relate constructively to diverse individuals and organizations; report writing and documentation.
• Driver's license and automobile liability insurance as required by law.

Responsibilities

• Gather data and produce initial and ongoing assessments.
• Present assessment data to Interdisciplinary Team for eligibility determination.
• Coordinate Person-Centered Planning with consumers and families.
• Counsel consumers regarding personal goals and services.
• Advocate to ensure provision of needed services and referrals.
