Gándara Center seeks a Family Partner to work with families in Fitchburg, MA, and Holyoke, MA. The role requires lived experience as a caregiver and bilingual English/Spanish or Portuguese communication to support families navigating child-serving systems and care plans.

The Family Partner will collaborate with a multidisciplinary team to mentor families, coordinate care, and link to community resources, contributing to improved treatment outcomes for youth and families.

Qualifications

• High school diploma or HISET required.
• Experience as a caregiver of youth with special needs preferred.
• Bi–lingual Spanish/English or Portuguese required.

Responsabilités

• Provide education and support during treatment and attend care coordination meetings.
• Assist caregivers in navigating child-serving systems and connecting to resources.
• Coordinate care via phone, in-person, or virtual meetings with relevant parties.
• Identify community resources and develop supports for youth and family.
