Job summary

Gándara Mental Health Center in New Bedford, MA seeks a Family Partner to work with families receiving clinical services. The role emphasizes lived experience to mentor families and help them navigate youth-serving systems and services.

The Family Partner educates and supports families, attends care coordination meetings, and connects caregivers to resources and peer supports to strengthen treatment outcomes.

Qualifications

• High school diploma or HISET required.
• Bi-Lingual Spanish/English or Portuguese strongly preferred.
• Must have a driver’s license and access to a vehicle.

Responsibilities

• Provide education and support throughout treatment, attend care coordination meetings, and assist caregivers in articulating youth strengths, needs, and goals.
• Educate caregivers about navigating child-serving systems and inform them about community resources.
• Support parents/caregivers in navigating systems and facilitating connections to peer/support groups.
• Provide care coordination via phone, in-person, or virtual meetings with collaterals, including referrals and warm hand-offs.
• Identify community resources and develop natural supports to sustain treatment goals.
• Collaborate with a multidisciplinary team to empower families to navigate the system independently.
