The Gandara Center in Lawrence, MA is hiring a Care Coordinator who is bilingual English to Spanish to support youth with serious emotional disturbance before age 21. You will coordinate services, develop individualized care plans, and work with a Family Partner to ensure culturally and linguistically appropriate care.

You will conduct home-based assessments, prepare progress notes, manage a caseload of 11-14 families, and collaborate with the Care Planning Team to build a comprehensive support

Formación

• Minimum 1 year of experience.
• Bilingual and bicultural encouraged (Portuguese/English or Spanish/English).
• Degree in Family Therapy, Counseling, Human Services or related field.
• Driver’s license and auto insurance required.
• Awareness of cultural and linguistic diversity.

Responsabilidades

• Submit detailed progress notes meeting regulatory and agency standards.
• Conduct home-based behavioral health assessments with MA CANS where applicable.
• Maintain a caseload of 11–14 families.
• Lead Care Planning Team and develop an Individual Care Plan and safety plan.
• Coordinate services and secure transportation as needed.
