Job summary

ServiceNet is seeking a Bi-Lingual Family Support Case Manager in Greenfield, MA. This full-time role involves supporting families with a member who has a disability, providing training, referrals, and community connections, and promoting empowerment across Hampshire and Franklin Counties.

The position requires a High School Diploma or GED, and bilingual proficiency in English and Spanish. Travel and driving are involved to support community-based services.

Qualifications

• Bilingual English/Spanish required.
• Strong communication and interpersonal skills.
• Experience in family-focused case work or disability services a plus.
• Knowledge of disability service resources is preferred.

Responsibilities

• Provide individualized support to families.
• Help develop family support plans and goals.
• Assist families in navigating available resources and services.
• Maintain ongoing contact and support as needed.
• Connect families with peer supports and community resources.
• Promote community integration and engagement.
• Help grow a network of support and information resources.
• Organize center-based and community activities and trainings.
