Job summary

Gándara Mental Health Center is seeking a Family Partner for the Springfield Family Resource Center. The FP reflects the community with lived experience, collaborating with clinicians to develop and implement Family Support Plans for CRA-related issues and coordinating referrals within the FRC network.

The role requires strong writing and interpersonal skills, bilingual Spanish/English preferred, and availability for weekend/evening hours.

Qualifications

• Lived experience or familiarity with CRA-related issues.
• Experience working with diverse populations in community settings.
• Available for weekend and evening hours.
• Valid driver's license and ability to meet driving history requirements.
• Strong interpersonal and writing skills; able to communicate effectively.
• Knowledge of Microsoft Office applications and Corel; proficiency with related office tech.
• Bilingual, especially Spanish/English, preferred.

Responsibilities

• Collaborate with Clinician to complete the CANS Assessment and develop a Family Support Plan.
• Develop and implement Family Support Plans based on identified needs; include tasks for family and agencies.
• Follow up with families to ensure connections to services and measure satisfaction within set timeframes.
• Review Family Support Plan at six months and adjust as needed.
• Provide outreach to community-based networks and coordinate referrals and services.
• Maintain confidential Family Support Plans and ensure proper record-keeping across systems.
