The Just One Project in North Las Vegas is seeking a DHRC Case Manager who will advocate for individuals and families, connect them to essential resources, and support stability and independence through individualized service plans. You will work directly with clients, documenting progress and coordinating with community partners in a compassionate, trauma-informed approach.

Qualifikationen

• Bachelor's degree or equivalent in Social Work or related field.
• Bilingual in English and Spanish is required.
• 3+ years of case management or related nonprofit experience.
• Experience with vulnerable populations and managing caseloads.
• Strong communication, organization, and problem-solving skills.
• Knowledge of trauma-informed care and client-centered service delivery.

Aufgaben

• Provide individualized case management, including assessments, service planning, crisis intervention, and ongoing support.
• Develop service plans and conduct monthly assessments to monitor progress and address barriers.
• Connect clients with housing, employment, education, healthcare, and legal resources.
• Assist clients with budgeting, savings, and financial literacy guidance.
• Coordinate safety planning and emergency services with supervisors and partners.
• Advocate for clients using a compassionate, trauma-informed approach.
• Maintain accurate case notes, referrals, and records in Salesforce within one business day.
• Update shared spreadsheets and submit weekly client success stories and reports.
