Job summary

Qualifications

• Bachelor's degree in a health or human service field required.
• Strong computer skills with knowledge of Microsoft Office programs.
• Self-motivated and able to work independently.
• Excellent organizational and communication skills.
• Willingness to travel around the counties.
• Bilingual Spanish required.

Responsibilities

• Oversee Early Intervention caseload of up to 50 cases and 75 units weekly.
• Monitor ongoing services and therapist performance across multiple therapies.
• Assist families in identifying evaluation or service providers.
• Follow NYS and NYC Regulations; timely paperwork submissions.
• Educate families about advocacy services.
• Coordinate services with medical and health care providers.
• Facilitate transition planning to preschool and other services when appropriate.
• Navigate web portals and databases; update and upload information for each child.
• Conduct home visits or in-person meetings as needed.
• Attend monthly supervision meetings for training and development.
• Perform other duties as assigned by manager.
