Job summary

Qualifications
- Bachelor's Degree in Social Work required.
- Experience coordinating IFSPs and services for infants/toddlers preferred.
- Willingness to travel extensively within assigned catchment areas.
Responsibilities
- Coordinate and develop Individualized Family Service Plans (IFSP) with families and professionals.
- Conduct home visits in the child’s natural environment to assess needs.
- Monitor progress and coordinate services with therapists and providers.
- Assist families in navigating services and advocate for necessary supports.
- Maintain accurate records and ensure regulatory compliance.
- Support the child’s transition from Early Start to services after age 3.
