Job summary

The Resource Exchange seeks a Service Coordinator, EI to guide eligible children through the Early Intervention process from referral to transition. You will work with families, document plans, and ensure services align with family priorities and regulatory timelines.

Responsibilities include coordinating evaluations, maintaining records, and collaborating with a transdisciplinary team across community locations. Strong communication and driving to sites are essential.

Qualifications

• Bachelor's degree or equivalent experience in the field.
• Experience with families and children with disabilities preferred.
• Excellent written and verbal communication skills.

Responsibilities

• Describing the Early Intervention Program to families in a way that is easy to understand and family friendly.
• Ensuring family rights are protected and timelines are met in accordance with state and federal regulations.
• Maintaining a complete and accurate child record for 55-65 children and ensuring databases are current.
• Coordinating evaluations and documenting results.
• Collaborating with team members to meet family priorities and support transitions.
