Job summary

Qualifications

• Bachelor's degree in social work, early childhood, human services or related field.
• Two years of experience providing case management, family support, or early intervention services.
• Experience coordinating or contributing to IFSP/IEP/service plans and working with multidisciplinary teams.

Responsibilities

• Develop and implement IFSP goals with families and the PCATT team.
• Provide coaching to strengthen family capacity and connect to resources.
• Maintain regular communication including home visits and follow-ups.
• Deliver family communication, coaching, and resource navigation in English and Spanish.
• Coordinate assessments, services, and transitions per Birth to Three requirements.
• Document services accurately across required systems and complete billing.
• Meet productivity expectations and collaborate with external providers.
