Job summary

NLACRC in Chatsworth, CA is seeking a Consumer Services Coordinator to provide information, advocacy, and service arrangement for clients and families in the SFV area.

The role involves developing program plans, coordinating services across providers, documenting per regulations, and attending interdisciplinary meetings. A BA in a human services field and 2+ years of experience are required; bilingual ability may be a plus.

Qualifications

• Bachelor’s degree in psychology, social work, sociology, or related human services field and at least two years of related experience. A Master’s degree in a related subject may be substituted for experience.
• Understanding of family systems and person-centered planning; empower individuals to achieve goals.
• Excellent verbal and written communication skills; proficient with MS Word and MS Outlook.

Responsibilities

• Provide information, advocacy, and service arrangement for consumers and families.
• Develop and monitor program plans; revise as needed.
• Identify and coordinate services for individuals and families.
• Document forms and reports per regulations and NLACRC policies.
• Advocate for individuals served by community agencies.
• Attend interdisciplinary meetings with individuals served, families, providers, advocates.
