Job summary

The Northeast Independent Living Program, Inc. in Lawrence, MA is seeking a Community Supports Specialist and Options Counselor to assist individuals with disabilities in making informed choices about long term care resources and services.

You will provide information, advocacy, and referral, helping clients navigate state and federal programs and complete applications. Key duties include counseling on long term care options, documenting interactions, coordinating with community agencies, and

Qualifications

• Associate degree in a Human Service or a related field and/or relevant independent living experience.
• Relevant experience in Health, Social Service, Education and/or Rehabilitation.
• Significant knowledge concerning MassHealth and its programs, services, and eligibility criteria.
• Proficient skills with technology including Microsoft 365, Teams, mobile devices, and applications.
• Strategic thinking, active listening, interviewing, and advocacy skills.
• Strong oral and written communication skills and excellent listening skills.
• Independent travel and reliable transportation required.

Responsibilities

• Provide non-biased counseling on the different options available for long term care.
• Offer information and educate the consumer about public, private and informal services and programs.
• Assist consumers in navigating state, federal and community services, programs and benefits.
• Support consumers in the application processes of benefits and housing programs.
• Discuss and gather information about the consumer’s resources and preferences.
• Engage in advocacy for the rights of persons with disabilities.
• Create and maintain case files and document visits and notes in the database.
• Participate in activities to help the ADRC gain visibility with community agencies.
