Qualifications

• Bachelor’s degree in Human Service related field preferred; or Associate’s degree with two additional years of relevant experience.
• Experience working with older adults and caregivers; health insurance knowledge preferred.
• Ability to communicate effectively, summarize data, prepare reports, and make recommendations.
• Good interpersonal skills and flexibility; team-oriented.
• Knowledge of person-centered counseling principles for older adults and caregivers.
• Reliable transportation and a valid driver’s license; some travel may be necessary.

Responsibilities

• Perform person-centered needs assessments to determine referrals.
• Develop preliminary care planning and long-term care counseling; gather information and follow up.
• Provide long-term care counseling in hospitals, nursing homes, or in-home settings.
• Assist with providing periodic public education sessions.
