Job summary

Soaring Eagles Home Care is seeking an Outreach and Community Engagement Coordinator to lead community engagement and education efforts within the Navajo and Hopi communities, focusing on high-potential service areas. This role builds culturally respectful relationships with community members, healthcare providers, and partner organizations to raise awareness of Medicaid/AHCCCS and ALTCS programs.

Qualifications

• Excellent verbal and written communication skills.

• Industry knowledge preferred; bilingual English/Navajo preferred.

Responsibilities

• Build trust and foster relationships with Navajo and Hopi communities and local organizations.
• Conduct outreach at events, provider sites, chapters, and local venues.
• Provide culturally sensitive education on Medicaid/AHCCCS and ALTCS programs, eligibility, and enrollment.
• Deliver in-services, presentations and distribute informational materials.
• Establish partnerships with IHS hospitals, planners, social workers, SNFs, ALTCS case managers, and other referrals.
• Serve as liaison between community members and providers to support transitions into care programs.
• Support referral workflows to ensure timely intake and communication with all parties.
• Track and document outreach activities, events, contacts, and referrals; report outcomes.
