Soaring Eagles Home Care is seeking an Outreach and Community Engagement Coordinator in Winslow, AZ. This part-time role focuses on engaging Navajo and Hopi communities, educating on Medicaid/AHCCCS and ALTCS, and coordinating referrals to ensure timely care access.

The ideal candidate will build trusted relationships with community members and providers, deliver culturally respectful outreach, and report on outcomes to support program improvement.

Qualifications

• Excellent verbal and written communication skills.
• Excellent interpersonal and customer service skills.
• Strong organizational skills with attention to detail and time management.
• Industry knowledge is highly preferred.
• Bilingual English/Navajo preferred.

Responsabilités

• Build trust and foster relationships with Navajo and Hopi communities and local organizations.
• Conduct outreach at events, provider sites, and community venues.
• Educate on Medicaid/AHCCCS and ALTCS eligibility and enrollment processes.
• Deliver in-services and distribute informational materials to raise program awareness.
• Establish partnerships with hospitals, planners, social workers, SNFs, and ALTCS case managers.
• Serve as a liaison between community members and providers for smooth transitions into care programs.
• Support referral workflows to ensure timely intake and communication with all parties.
• Track outreach activities, events, contacts, and referrals; report outcomes and trends.
