Job summary
Huron Regional Medical Center is seeking a Community Health Worker to help bridge gaps between communities and health and social services. You will provide one-on-one support to individuals and families, enhance communication with healthcare resources, and assist in navigating the health care system, while practicing cultural competency and integrity.

Qualifications
- Certification or willingness to obtain Community Healthcare Worker from an accredited college
- Must have a valid driver's license and reliable personal transportation
- Familiar with community resources, 211 program, and local healthcare services
- Excellent written, verbal, and non-verbal communication skills
Responsibilities
- Provide one-on-one support to individuals and families to bridge gaps in care
- Enhance communication between community members, healthcare resources, organizations, and businesses
- Assist individuals in navigating the health care system with cultural competency
