Job summary

Huron Regional Medical Center is seeking a Community Healthcare Worker to bridge the gap between the community and health and social service systems. You will provide one-on-one support to individuals and families and help navigate healthcare resources.

The role requires familiarity with local resources (including the 211 program) and strong communication skills. A valid driver’s license and reliable transportation are required; a competitive benefits package is offered for full-time employees.

Reports To: Director of Social Services

Status: 80 hours per two week pay period

Shift: 8:00 am – 4:30 pm

Job Schedule: Full time

Weekly Hours: 40 hours

HRMC is looking for applicants to help bridge the gap between the community and health and social service system by providing one on one support to individuals and families. Enhance communication between community members , healthcare resources , organizations, and businesses through informal education, prescription assistance and assist in navigating through the health care system. This position will maintain and practice cultural competency and integrity within the community.

Requirements

• Certification or willing to obtain Community Healthcare Worker from an accredited college

Working Conditions:

• Work indoors will be in clean, well-lighted, climate-controlled environment with minimal hazards under normal conditions.
• Work will require travel to patient homes which could expose you to a variety of settings.
• Work requires finger/hand dexterity and eye/hand coordination along with accurate keyboarding skills.
• Position involves sitting, standing, keyboarding, filing, occasional light lifting, walking, stooping, squatting, and bending.
• Risk exposure to communicable disease.
