Job summary

Huron Regional Medical Center in Huron, SD is seeking a Community Health Worker to bridge the gap between families and the health and social services system.

You will provide one-on-one support, connect clients with resources, assist with navigating care, and educate the community while maintaining cultural competency. Eligible candidates have certification or willingness to obtain CHW credentials, a valid driver's license, and strong communication skills.

Qualifications

• Certification or willingness to obtain Community Healthcare Worker certification from an accredited college.
• Must have a valid driver's license and reliable personal transportation.
• Familiar with community resources, 211 program, and local healthcare services.
• Excellent written, verbal, and non-verbal communication skills.

Responsibilities

• Bridge the gap between the community and health and social service systems by providing one-on-one support.
• Enhance communication between community members, healthcare resources, organizations, and businesses through informal education.
• Assist in navigating the healthcare system and reviewing benefit eligibility for clients.
• Maintain cultural competency and integrity within the community.
