Job summary

Qualifications

• High school diploma or equivalent.
• CHW training and certification.
• Minimum of five (5) years' experience in community health, social services, outreach or related fields.
• Understanding of the local community within the service delivery area.
• Private, reliable transportation.
• Bi-lingual (English/Spanish) required; Spanish tested prior to hire.

Responsibilities

• Outreach, assessment, and member education activities.
• Coordinate services and provide member support.
• Monitor a caseload of Star Kids members not eligible for LMSW/RN roles.
• Maintain deadlines and manage competing priorities.
