Job summary

Behavioral Health Solutions of South Texas is seeking a Community Health Worker to empower individuals and families to access behavioral health services and community resources. You will promote program services, build partnerships, and help reduce barriers to care in the Pharr, TX area.

The role involves outreach, referrals, and care navigation with documentation and grant compliance. A Texas CHW certification or the ability to obtain it within 6 months is required or preferred, with bilingual

Qualifications

• High School Diploma, GED, or experience equivalency.
• At least 2 years of experience in social services or with at-risk populations.
• Texas driver’s license, reliable transportation, and liability insurance.
• Bilingual in English and Spanish preferred.
• Current Texas CHW Certification or ability to obtain within 6 months.

Responsibilities

• Conduct community outreach, presentations, and networking to engage individuals with trauma, substance use, or behavioral health risk factors.
• Build and maintain partnerships with community organizations; develop MOUs, coordinate referrals, identify barriers to access.
• Screen individuals using ACEs and SDOH tools to determine service needs.
• Connect participants to behavioral health, healthcare, and community resources with referrals and follow-ups.
• Maintain documentation and ensure compliance with organizational policies and grant requirements.
