Responsibilities

• Attend, organize, and represent Via Care at community events, health fairs, and local centers.
• Educate residents about Via Care services and assist with enrollment and onboarding.
• Coordinate with enrollment staff and mobile units to promote seamless onboarding.
• Track outreach activities and report on patient engagement and enrollment outcomes.
• Build and maintain partnerships with community organizations and referral sources.
• Travel across service areas and attend evening or weekend events as needed.
