Job summary

Via Care Community Health Center is seeking an Outreach and Enrollment Specialist to engage communities, educate residents about available health services, and support enrollment and onboarding at the Los Angeles service area.

You will build partnerships with schools, nonprofits, and local organizations, coordinate with mobile units and clinics, and help reduce barriers to care while promoting access to primary, dental, behavioral health, women's health, senior services, and urgent care.

Qualifications

Responsibilities

• Attend, organize, and represent Via Care at community events and health fairs.
• Build and maintain partnerships with community organizations and referral sources.
• Educate community members about healthcare services and assist with enrollment and onboarding.
• Coordinate with enrollment staff and clinic operations to promote seamless onboarding.
• Track outreach activities and report on partnerships and engagement metrics.
