Job summary

OneWorld Community Health Centers in Omaha, NE seeks a Values Based Care Community Health Worker (VBCCHW) to serve as frontline public health professional, liaising between the health center and communities served. You will conduct outreach, provide culturally appropriate health education and coaching, coordinate care, and support program implementation.

Qualifications

• High school diploma or equivalent required.
• Proficiency in English required; bilingual Spanish/English preferred.
• Valid driver's license, current auto insurance, and MVR as per policy.
• Ability to work with culturally diverse populations.
• Willingness to travel to community sites and partner locations.
• Documented CHW training or equivalent experience in outreach/public health.

Responsibilities

• Conduct community outreach through home visits, events, and partnerships.
• Provide culturally and linguistically appropriate health information and coaching.
• Coordinate care and facilitate referrals to clinical and social services.
• Advocate for access to healthcare, housing, food security, and transportation.
• Serve as liaison between health center and community, communicating needs.
