Job summary

OneWorld Community Health Centers, Inc. seeks a full‑time Value Based Care Community Health Worker to connect with diverse communities and promote wellness.

You will work under a supervisor to provide health education, navigate health and social services, and coordinate referrals while addressing social determinants of health. Responsibilities include home visits, community outreach, advocacy, and acting as a liaison to clinical and administrative teams.

Qualifications

  • High school diploma or equivalent required.
  • Proficiency in English required.
  • Bilingual in Spanish and English required; additional languages valued.
  • Valid driver’s license, current auto insurance, and MVR per policy.
  • Documented CHW certification or equivalent experience in community health.
  • Knowledge of cultural, social, and health issues affecting the communities served.
  • Experience with data entry, EHRs, or client tracking systems.
  • Lived experience or deep familiarity with the community served.

Responsibilities

  • Conduct outreach through home visits, events, and partnerships with local organizations.
  • Provide culturally and linguistically appropriate health information and coaching.
  • Coordinate care and facilitate referrals to clinical services and social supports.
  • Advocate for access to healthcare, housing, food security, and transportation.
  • Serve as liaison between OneWorld and the community; relay needs to teams.
  • Support program implementation and monitor grant and regulatory requirements.

Job description

Two full-time positionsJob DutiesThe Value Based Care Community Health Worker (VBCCHW) serves as a frontline public health professional and trusted liaison between OneWorld Community Health Centers and the communities we serve. Working collaboratively under the direction of their assigned Program Manager or Supervisor, the VBCCHW promotes wellness by helping individuals and families adopt healthy behaviors, navigate health and social service systems, and overcome barriers to care. The VBCCHW conducts outreach, provides culturally appropriate health education and coaching, coordinates care, advocates for community needs, and supports program implementation aligned with grant, organizational, and regulatory requirements.Conducts community outreach through home visits, door-to-door canvassing, community events, health fairs, and partnerships with local organizations and businesses.Provides culturally and linguistically appropriate health information to individuals, families, and community groups on assigned health topics.Coordinates care among individuals, communities, and health and social service systems, including facilitating referrals to clinical services, social supports, and community resources.Provides coaching and social support to individuals and families, including but not limited to nutrition, chronic disease self-management, preventive health practices, and health screenings.Advocates for individuals and communities regarding access to healthcare, housing, food security, transportation, and other social determinants of health.Serves as a liaison between the health center and the community, communicating community needs to clinical and administrative teams and ensuring organizational responsiveness.The tasks outlined above are intended to provide a general overview of the role and are not an exhaustive list of all duties, responsibilities, or job functions.
