Job summary

Community Action Partnerships of Western Nebraska (CAPWN) seeks a Community Health Worker to help patients and families access health and community resources, working with the Primary Care team to promote healthier behaviors. The CHW will conduct intake, provide education, and assist with care plans in a culturally sensitive manner.

The role emphasizes motivational interviewing, follow-up, and coordination with a multidisciplinary team.

Qualifications

• High school diploma or GED required; bilingual Spanish/English.
• Written and oral fluency in Spanish and English; experience in multicultural settings preferred.
• CPR certification within 30 days of onboarding; driving record acceptable; basic computer skills.
• Experience with Electronic Medical Records (EMR) and office procedures is helpful.

Responsibilities

• Assist patients and families in navigating health and community resources.
• Participate in integrated Primary Care team-based care model.
• Provide culturally appropriate health education and self-management support.
• Conduct intake interviews to identify barriers to health and follow up via calls or visits.
• Educate patients individually or in groups and facilitate referrals to community resources.
