• Experience working with underserved, low-income or culturally diverse populations.

Responsibilities

• Outreach to members and engage with services.
• Support care coordination and accompany home visits as needed.
• Connect members to community resources and schedule appointments.
• Document outreach and referrals in care management system.
• Assist with applications for Medi-Cal, CalFresh, and other entitlements.
