Qualifications

• Bilingual Spanish required.
• Experience with housing resources, benefits, and social determinants of health preferred.

Responsibilities

• Serve as primary contact for social services members to navigate health care systems.
• Maintain an active caseload of ~35 patients with care coordination.
• Conduct comprehensive psychosocial assessments and develop individualized care plans.
• Coordinate services with MAT, HCH, and ECM teams to address housing and social needs.
• Educate patients about housing resources and public benefits; assist with required documents.
