Job summary

Community Clinic Inc. in Silver Spring, MD seeks a Community Health Worker I to assist newly arrived refugees and other visa holders access health and social services. This full-time role requires bilingual abilities, strong communication, and guidance through the healthcare system.

Responsibilities include patient intake, enrollment referrals, and coordinating with the clinical team to support care management. Travel between sites may be required; a commitment to health equity is essential.

Qualifications

• High School diploma or equivalent.
• Completion of an accredited CHW program.
• Maryland CHW Certification required or must attain within 90 days of employment.
• Current BLS/CPR certification required.
• Previous experience as a Community Health Worker preferred.
• Cultural competence and ability to work with diverse populations.
• Excellent communication and interpersonal skills.
• Proficiency in languages commonly spoken within the refugee population.

Responsibilities

• Provide education on preventive care, health promotion, and disease management to individuals and families within the target population.
• Assist individuals in navigating the healthcare system, scheduling appointments, and accessing necessary resources and services.
• Support the clinical team by assisting in screening, assessing, and completing other health-related tasks as needed.
• Establish trusting relationships with patients and their families and provide general support and encouragement.
• Conduct intake interviews with patients, including enrolling patients into, and/or referring them to, various health, assistance, and social service programs.
• Follow-up with patients by phone, home visits, and visits to other settings where patients can be found.
• Assist patients with completing applications and registration forms.
• Conduct and verify eligibility determination, enrollment, and follow-up with uninsured patients.
• Help patients connect with transportation resources and give appointment reminders in special circumstances.
• Be knowledgeable about community resources appropriate to the needs of patients/families.
• Act as a patient advocate and liaison between the patient/family and community service agencies.
• Record patient care management information in the EMR (training provided) and other software no later than 24 hours after patient contact.
• Attend regular staff meetings, training, and other meetings as requested.
• This position may require travel between CCI sites to provide additional coverage when needed.
