Job summary

Chicanos Por La Causa, Inc. is seeking a Community Navigator to help families access health coverage and essential services in the Phoenix area.

You will connect individuals with AHCCCS, SNAP, health insurance marketplace plans, and local resources, guiding them through applications and referrals with care and cultural competence. Fluency in English and Spanish is required; two years in healthcare or related field; valid driver’s license; and a passion for community well-being.

Qualifications

• Minimum of two years of experience working in a healthcare setting or related field.
• Fluency in English and Spanish, including speaking, reading, and writing.
• Knowledge of local population demographics, community resources, assets, and needs.
• Knowledge of health insurance programs, including Medicaid, KidsCare, and the Health Insurance Marketplace.
• Valid driver's license and current automobile insurance.

Responsibilities

• Connect individuals and families with essential resources, including health insurance, food assistance, housing, and healthcare information.
• Assist with completing electronic applications for AHCCCS through Health-e-Arizona PLUS and SNAP, with referrals to additional resources.
• Help individuals understand, apply for, and enroll in health coverage through the Health Insurance Marketplace.
• Engage in community outreach and health education to empower informed health decisions.
