Greater Lawrence Family Health Center, Inc. seeks a bilingual Community Health Worker to join the Community Support Services Team, coordinating care and coordinating referrals for a diverse patient population across multiple sites in Lawrence, Methuen, and Haverhill.

The CHW will provide linguistic services, assist with eligibility, and accompany patients to medical and behavioral health appointments, ensuring follow-up through various outreach methods.

Qualifications

• Two years’ experience in CHW or related field.
• Bilingual Spanish/English; strong communication.
• Proficiency with Word, Excel, Outlook.
• Massachusetts driver’s license and reliable transportation.

Responsibilities

• Provide information about community services and assist with eligibility.
• Assist with referrals and coordinate care.
• Interpret and translate to support linguistic services at appointments.
• Accompany patients to appointments when needed.
• Maintain follow-up contact through calls and home visits.
• Collaborate with departments and share patient social context with staff.
