Job summary

The Greater Lawrence Family Health Center is seeking a bi-lingual (Spanish/English) Nurse Practitioner to join our Patient Centered Healthcare Team. You will assess patients, manage chronic conditions, and collaborate with a multidisciplinary staff to deliver high-quality care.

Reporting to the Medical Director, this role requires MA licensure, at least 2 years of NP experience, and the ability to work across sites with travel as needed.

Qualifications

• At least 2 years of NP experience.
• Licensed in Massachusetts.
• Bi-lingual (Spanish) required.

Responsibilities

• Assess patients including medical history, labs, and social factors.
• Develop and implement plans for chronic illness management and acute care.
• Collaborate with multidisciplinary team to coordinate patient care.
• Educate patients and families and document teaching.
• Travel between sites may be required.
