Greater Lawrence Family Health Center, Inc. is seeking a CSS, Nurse Care Manager to support Community Based Services. This role focuses on assessing, planning, and coordinating patient care with a multidisciplinary team, including education and counseling on prevention and harm reduction.

The position involves in-person care with some remote options, requires MA RN licensure and CPR, and mandates adherence to HIPAA and program policies. Travel between sites is expected.

Qualifications

• Massachusetts RN license (unexpired) is required.
• Minimum 2 years of nursing care experience.
• Bilingual Spanish/English strongly preferred.
• CPR certification required.
• Care management or care coordination experience preferred.
• Knowledge of care for diverse populations including those with substance abuse.
• Strong communication, organization, and accountability.

Responsibilities

• Lead and participate in CSS care management activities with a multi-site team.
• Perform follow-up nursing visits and administer medications.
• Educate patients about disease states, prevention, and adherence.
• Coordinate care with PCPs, specialists, pharmacists, and CHW.
• Ensure HIPAA compliance and maintain EMR documentation.
• Travel between GLFHC sites as needed.
