Job summary

Greater Lawrence Family Health Center (GLFHC) seeks a bilingual Nurse Practitioner (Spanish/English) to deliver high-quality care across multiple sites in the Merrimack Valley. Reporting to the Medical Director, you will be a key member of the Patient Centered Healthcare Team.

Your duties include assessing patients, reviewing labs, planning treatment, educating patients and families, coordinating with case managers and other providers, and ensuring culturally competent care.

Qualifications

• Bilingual Spanish/English language skills required.
• Ability to work effectively within a multidisciplinary team.
• Strong patient communication and clinical assessment skills.

Responsibilities

• Assess patients including medical history, labs, and living situation.
• Review abnormal labs and plan disposition/treatment with minimal supervision.
• Develop and implement care plans, educate patients and families, and coordinate with other providers.
• Collaborate with case managers and the healthcare team; involve family when appropriate.
• Travel between GLFHC sites may be required.
