Job summary

Greater Lawrence Family Health Center, Inc. is seeking a bi-lingual Nurse Practitioner to deliver high-quality care across our multi-site system in Lawrence, MA. The NP will assess patients, manage chronic and acute conditions, and collaborate with a multidisciplinary team to optimize outcomes.

Requirements include at least 2 years as a Nurse Practitioner, Massachusetts NP license, and fluent Spanish. Some travel between GLFHC sites may be required to meet patient needs.

Qualifications

• Minimum 2 years of recent Nurse Practitioner experience.
• Licensed in Massachusetts as NP.
• Bi-lingual (Spanish) required.
• Ability to work in a multi-disciplinary team.

Responsibilities

• Assess patients including presenting problem, medical history, allergies, psychosocial factors, medications, and living arrangements.
• Review labs and plan disposition/treatment when primary physician is unavailable.
• Identify teaching needs and referrals; develop a patient education plan.
• Collaborate with case managers and other providers; involve family as appropriate.
• Provide care per approved protocols and document patient education and responses.
• Contribute to teaching and orientation of staff; share knowledge.
• Travel between clinical sites may be required.
