Job summary

Providence Housing Authority is seeking a Community Health Worker to connect residents of public housing and Section 8 participants to health and social services, provide social support, and advocate for their health needs. The role focuses on safety, healthy housing, and community empowerment.

You will work one-on-one with residents, assess needs, and document progress while collaborating with RSD Wellness Team to improve health outcomes and housing conditions in Providence.

Qualifications

• High school diploma or GED required.
• CHWARI CORE Community Health Worker Certification required.
• Fluency in English and Spanish (spoken and written) required.
• Experience in customer service, social services, health care or community work preferred.

Responsibilities

• Work one-on-one with residents to address health concerns.
• Assess healthcare and social service needs and refer to services when possible.
• Set health-promoting goals with residents and help them meet these goals.
• Document all client progress in the case management database.
• Maintain confidentiality of client information, processes, and data.
• Meet regularly with the Resident Services Department Wellness Team and other staff for program development and evaluation.
• Communicate with community partners, service providers and PHA staff.
• Attend CHWARI networking meetings monthly.
• Network with health and community-based organizations to learn about opportunities for clients.
• Support the work of Resident Services Department staff and Property Management to address social determinants of health.
