Qualifications

• High School Diploma or GED completed.
• Bilingual – English and Spanish.
• Lived experience in the community served.
• Ability to become a certified CHW.

Responsabilités

• Help members navigate and access medical care and community services.
• Promote, maintain, and improve health through social support and advocacy.
• Make follow-up calls and community visits to connect members to services.
• Meet members in medical settings to ensure coordinated care.
• Document member encounters in the documentation platform.
• Maintain relationships with members, families, providers, and CBOs.
