Job summary

Axis Community Health is seeking a Community Health Worker to advocate for patients and provide case management across medical, mental health, and social support needs. The role may involve home visits and off-site patient interactions, requiring independence, reliability, and travel to multiple sites.

Qualifying candidates will have a high school diploma (AA/BA preferred), 1 year CHW experience in behavioral health settings, and strong communication skills.

Qualifications

• High school diploma or equivalent required; AA/BA in Public Health or related field preferred.
• 1 year CHW experience in integrated behavioral healthcare is preferred.
• Strong written and verbal communication; bilingual English/Spanish preferred.
• Valid driver's license and reliable transportation required.
• Ability to learn quickly, adapt to change, and work independently.

Responsibilities

• Advocate for patients and facilitate communications with healthcare providers.
• Conduct outreach and enrollment assessments for health programs.
• Educate community members on health topics with care-team support.
• Perform health screenings including social determinants of health.
• Develop care plans and coordinate connections to resources.
• Collaborate with local organizations to enhance community partnerships.
• Educate patients about Axis services and ensure proper documentation.
• Travel to other sites as needed to perform duties.
