Job summary

Suvida Healthcare LLC is seeking a Bilingual Community Health Worker to serve Medicare-eligible Hispanic seniors in the Katy, TX area. The Guia will coordinate patient care, engage families, and connect patients with social and community resources in a collaborative team setting.

The role requires strong communication, cultural competency, and the ability to work across home, clinic, and remote environments. Certification and a preferred Bachelor’s degree are valued.

Qualifications

• Experience in healthcare settings, especially with seniors.
• Bilingual in English and Spanish.
• Ability to work at home, in-clinic, and remotely.

Responsibilities

• Coordinate comprehensive care for assigned patient panel.
• Collaborate with care teams and external partners to address social needs.
• Facilitate referrals and ensure closed-loop communication with patients and families.
• Document interventions in patient records across settings.
• Onboard patients to Suvida model and schedule care visits.
• Support social determinants of health optimization for patients.
