Job summary

Felton Institute is seeking a Bilingual Clinical Case Manager to support AB 109 clients with clinical consultation, triage, and care coordination across ACPD, ACBHD, and community providers. The role emphasizes trauma-informed, strength-based services and collaboration with Probation Officers, peer supports, and in-custody providers.

Responsibilities include information gathering, service planning, barrier removal, and outreach to hard-to-reach clients.

Qualifications

• Master’s degree in social work, Counseling, Marriage and Family Therapy, Psychology, or a closely related behavioral health field from an accredited institution.
• Registered as an Associate (ASW, AMFT, or APCC) with the California Board of Behavioral Sciences, or actively licensed (LCSW, LMFT, or LPCC).
• Minimum two (2) years of post-graduate clinical or case management experience in mental health, substance use, or co-occurring disorder services.
• Bilingual in Spanish/Cantonese/Mandarin is REQUIRED.
• Experience with justice-involved adults, including PRCS, Penal Code 1170(h), formal probation, or specialty court populations (strongly preferred).
• Experience navigating Medi-Cal, managed care plans, private insurance, ACCESS, and community-based behavioral health systems.

Responsibilities

• Provide clinical consultation and triage to DPOs and AB 109 clients on mental health, substance use, and co-occurring disorder services.
• Review client clinical history via intake forms, SHIE, and other records.
• Respond to referrals within 24 business hours and complete client outreach within 72 hours.
• Triage clients to appropriate services within 60 days of referral activation.
• Coordinate discharge planning and pre-release activities with partner agencies and facilities.
• Develop HIPAA-compliant Provider Service Plans within 30 days of engagement; submit closing summaries for immediate-triage clients.
