Job summary

Felton Institute seeks a Clinical Case Manager to manage AB 109 caseload, provide clinical consultation, assessment, care planning, and outreach.

You will collaborate with Deputy Probation Officers, peer supports, and community providers to deliver trauma-informed, evidence-based services, with on-site work in Oakland.

This full-time role offers a competitive salary range and benefits; requires a Master’s degree and California licensure or Associate registration.

Qualifications

• Master’s degree in social work or related field.
• Current California BBS license or associate registration.
• Minimum two years of post-graduate clinical or case management experience.
• Experience with justice-involved adults (preferred).
• Experience navigating Medi-Cal and insurance systems.

Responsibilities

• Provide clinical consultation and triage for AB 109 clients.
• Coordinate case planning and discharge planning with partners.
• Conduct information gathering from in-custody providers and hospitals.
• Document services in the ACPD Provider Portal within 72 hours.
• Engage clients and remove barriers to services.
