Qualifications

• Minimum of 2 years in case management, substance use disorder and mental health treatment for TAY and adults, including co-occurring disorders.

Responsibilities

• Provide care and case management, mental health and substance use recovery, and crisis prevention within a multidisciplinary team.
• Utilize evidence-informed interventions to address guests' needs and goals.
• Collaborate with community partners as access and intervention points.
• Meet documentation deadlines and comply with Medi-Cal requirements when applicable.
• Connect guests with housing, transportation vouchers, emergency food, and hygiene products.
• Link guests to primary care and other health services.
• Develop gender- and culturally-responsive curriculum.
