Qualifications

• Bachelor's degree or 2 years supervisory experience in a home healthcare role.
• Demonstrated customer service and communication skills.
• Ability to speak English and Spanish fluently.

Responsibilities

• Serve as the first point of contact for clients, partners, and care providers with excellent service.
• Take intake calls and gather information for client care/plan and communicate service details and fees.
• Monitor dashboards daily and follow up with caregivers and clients.
• Share on-call responsibilities every other month as needed.
• Input care plans, ensure staffing, and review information with caregivers before care starts.
• Complete monthly client calls and document in the file.
• Update care plans, upload monthly reports, and communicate changes to caregivers.
• Match caregivers to clients for best fit and ensure plan compliance.
• Maintain accurate documentation of all care plan tasks and participate in meetings.
• Perform clerical tasks and other duties as assigned.
