Catholic Charities, Archdiocese of New York is seeking a Senior Case Manager to support individuals and families in Westchester and the Bronx through comprehensive case management, crisis intervention, and advocacy.

This full-time role requires a Bachelor's degree, 1–2 years’ experience, bilingual English/Spanish, and strong collaboration with community partners to connect clients with housing, food, and social services.

Qualifications

• Bachelor's degree in social services required.
• 1–2 years of case management and social services experience.
• Bilingual English/Spanish required.

Responsibilities

• Provide advanced case management for a broad population of individuals and families in the Bronx and Westchester.
• Engage clients, assess needs, plan services, refer, and follow up with a client-centered approach.
• Provide crisis intervention in emergencies around shelter, food, clothing, and other basic needs through financial assistance, advocacy, benefits access and resources.
• Develop service plans addressing immediate needs and long-term goals.
• Build and maintain collaborative relationships with community agencies, other Catholic Charities Departments, Divisions, Affiliates and Archdiocesan entities.
• Help develop and maintain access to a wide range of social service benefits and resources.
• Enter data into two databases and meet program deliverables; document for funders and stakeholders.
• Participate in weekly supervision and agency meetings; attend community meetings and trainings.
• Other duties as assigned.
