Job summary

Catholic Charities, Archdiocese of New York is seeking a Senior Case Manager to support individuals and families in Westchester and the Bronx through comprehensive case management, crisis intervention, and advocacy.

Responsibilities

• Provide advanced case management for a broad population of individuals and families in the Bronx and Westchester.
• Engage clients, assess needs, plan services, refer, and follow up with a client-centered approach.
• Provide crisis intervention in emergencies around shelter, food, clothing, and other basic needs through financial assistance, advocacy, benefits access and resources.
• Develop service plans addressing immediate needs and long-term goals.
• Build and maintain collaborative relationships with community agencies, other Catholic Charities Departments, Divisions, Affiliates and Archdiocesan entities.
• Help develop and maintain access to a wide range of social service benefits and resources.
• Enter data into two databases and meet program deliverables; document for funders and stakeholders.
• Participate in weekly supervision and agency meetings; attend community meetings and trainings.
• Other duties as assigned.

Summary: This unique position's primary function is to fulfill Catholic Charities commitment to serve individuals and families in an inclusive and holistic manner. An integral part of the Case Management team, this position will be based in Westchester. The position provides direct assistance for those in need through comprehensive case management services.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

• Provides an advanced level of case management to a broad population of individuals and families in the Bronx and Westchester county.
• Provides effective engagement, assessment, service planning, referral and follow-up using a client-centered approach.
• Provides crisis intervention in emergencies around shelter, food, clothing, and other basic needs through the provision of financial assistance, goods, advocacy, benefits access and access to relevant resources.
• Consistently conducts thorough assessments and develops a service plan that addresses immediate needs and longer-term goals.
• Develops knowledge about and maintains collaborative relationships with community agencies, other Catholic Charities Departments, Divisions, Affiliates and Archdiocesan entities.
• Develops a capacity to provide or ensure access to a wide range of social service benefits and helps develop and maintain a network of high-quality resources.
• Fulfills operational and other duties.
• Enters all information into two separate database platforms, meets program deliverables in a timely and efficient manner, and understands the significance of documentation to funders and other stakeholders.
• Actively participates in weekly supervision.
• Attends and participates in monthly Department meetings, community meetings, trainings, and other agency-related activities.
• Other duties as assigned

Position Type and Expected Hours of Work:

This is a full-time position. Days and hours of work are generally 9:00 AM to 5 PM Monday - Friday. Additional hours may be required to meet program deadlines, or client needs. There may be periods when work hours vary.

Working conditions and physical demands required:

The demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

• Remain in a stationary position at a workstation and use a computer approximately 60% of the time.
• Travel within Westchester may be required, as needed.

Qualifications

Education and/or experience required:

• Bachelor's degree required in the social services field with at least 1-2 years of experience in case management and social services.
• Bilingual English/Spanish required.

Skills, Licenses, and/or competencies required:

• Excellent interpersonal skills; the ability to work well independently and as part of a team.
• Knowledge of social service delivery systems and local resources.
• Excellent organizational, time-management and communication skills.
