Job summary

Pierce County is seeking a bilingual Vietnamese-speaking Case Manager in the Human Services department. You will manage a caseload, assess needs, authorize and monitor care plans for older adults and adults with disabilities in home settings.

The role offers a hybrid work arrangement, with in-office start and telework thereafter through probation. A strong emphasis on communication and independence supports client safety and independence.

Qualifications

• Bachelor's degree in social work, gerontology, psychology, counseling and guidance, sociology or related field.
• Two years or more of direct human services experience, preferably with seniors.
• Master's degree in behavioral health sciences.
• One year of social services experience preferred.
• Bilingual and fluent in Vietnamese required; interview will include Vietnamese questions.

Responsibilities

• Assess, coordinate services, and advocate for older and disabled adult clients.
• Provide formal reassessment at specific intervals to gauge progress and continuing needs.
• Develop, implement, and monitor service plans through regular contact with clients and service providers; participate in related case staffing.
• Must be able to travel to assigned work locations, including field assessments.
