Job summary

Qualifications

• Bachelor's degree in social work, gerontology, psychology, counseling and guidance, sociology or related field.
• Two years or more experience providing direct human services.
• Master's degree in behavioral health sciences.
• One year of experience in the social services field preferred.
• Bilingual and fluency in Samoan required; interview in Samoan.

Responsibilities

• Assess, coordinate services, and advocate for older and disabled clients.
• Provide formal reassessments at specific intervals to gauge progress and continuing needs.
• Develop, implement, and monitor service plans with regular contact with clients and service providers; participate in related case staffing.
• Must be able to travel to assigned work locations, including field assessments.
