Job summary

Pierce County Human Services is seeking a bilingual Vietnamese Case Manager to support older adults and adults with disabilities in Medicaid services. You will assess needs, authorize and monitor care plans, and deliver in-home case management with a laptop.

This role directly impacts clients’ safety, independence, and quality of life. The position emphasizes field work with home visits, a strong communication ethos, and collaboration with service providers while adhering to program policies.

Qualifications

• Bachelor's degree in social work, gerontology, psychology, counseling and guidance, sociology or related field.
• Two years or more of experience providing direct human services, preferably in the area of providing services to senior citizens, is required.
• Master's degree in behavioral health sciences AND One year of experience in the social services field preferred.
• Bilingual and fluency in Vietnamese in all aspects of communication is required. Part of the interview process will involve questions in Vietnamese with someone who is also bilingual.

Responsibilities

• Responsible for assessment, coordination of services, and advocacy of older and disabled adult clients.
• Provide formal reassessment at specific intervals to gauge progress and continuing needs.
• Develop, implement, and monitor service plans through regular contact with clients and service providers; participate in related case staffing.
• Must be able to travel to assigned work locations, including but not limited to field assessments.
