Job summary

Pierce County Human Services in Washington is seeking a bilingual Case Manager fluent in Samoan to support older adults and disabled clients in Medicaid services. You will assess needs, authorize services, and monitor care plans, often working with a laptop in clients' homes.

You will help connect clients with essential services to ensure safety and independence at home. This grant-funded role emphasizes compassionate, client-centered support and collaboration with partners.

Qualifications

• Bilingual capability in Samoan is required.
• Experience in case management and Medicaid program familiarity preferred.

Job description

This position is eligible for 10% field work specialty pay and 5% bilingual pay for hours worked.

Hybrid - The first 3 months in-office, then scheduled levels of telework until probation is complete.

Special Note:

This is a grant-funded position. Continued employment is contingent upon job performance/evaluation, continued funding of the program, and other appropriate County procedures.
