Job summary

Pierce County Human Services seeks a bilingual Case Manager fluent in Vietnamese to assess needs, authorize and oversee care plans for older and disabled clients. You’ll work with clients in their homes and coordinate with providers, using a laptop for documentation.

You will join a team dedicated to helping seniors and people with disabilities stay safe and independent at home, with a direct impact on quality of life. Strong communication is essential, both written and verbal.

Qualifications

  • Bachelor's degree in social work, gerontology, psychology, counseling and guidance, sociology or related field.
  • Two years or more of direct human services experience, preferably with senior citizens.
  • Master's degree in behavioral health sciences.
  • One year of experience in the social services field preferred.
  • Bilingual in Vietnamese is required; interview will be conducted in Vietnamese.

Responsibilities

  • Responsible for assessment, coordination of services, and advocacy of older and disabled adult clients.
  • Provide formal reassessment at specific intervals to gauge progress and continuing needs.
  • Develop, implement, and monitor service plans through regular contact with clients and service providers; participate in related case staffing.
  • Must be able to travel to assigned work locations, including but not limited to field assessments.

Job description

This position is eligible for 10% field work specialty pay and 5% bilingual pay for hours worked.

Hybrid - The first 3 months in-office, then scheduled levels of telework until probation is complete.

This is a grant-funded position. Continued employment is contingent upon job performance/evaluation, continued funding of the program, and other appropriate County procedures.

How To Be Successful In The Role

We are looking for someone with case management experience who is diligent, detail-oriented, and a strong communicator. You will use your exceptional written and verbal communication skills frequently with clients, family members, and community providers to ensure services are being provided in accordance with the established care plan. We need someone who is prompt with responses and can act with a sense of urgency, as well as work well independently and as part of a team.

Minimum Qualifications
  • A Bachelor's degree in social work, gerontology, psychology, counseling and guidance, sociology or a related field AND
  • Two years or more of experience providing direct human services, preferably in the area of providing services to senior citizens, is required.
  • Master's degree in behavioral health sciences AND
  • One year of experience in the social services field preferred.
  • Bilingual and fluency in Vietnamese in all aspects of communication is required. Part of the interview process will involve questions in Vietnamese with someone who is also bilingual.
Special Requirements And/or Qualifications
  • A valid driver’s license and reliable transportation is required.
